Agilon Health announced on February 28 that it acquired Family Practice Center, PC.

Starting in 2024, the partnership will transition Family Practice Center to a full-risk, value-based care model for central Pennsylvania Medicare patients. Terms of the deal were not disclosed.

With more than 190 physicians, Family Practice Center, PC is one of central Pennsylvania’s largest independent primary care physician groups. It serves more than 200,000 patients across 42 locations in 12 counties.

Agilon Health is transforming health care for seniors by empowering primary-care physicians to focus on the entire health of their patients. Agilon is accelerating at scale how physician groups transition to a value-based Total Care Model for senior patients.

According to data captured in the LevinPro HC database, this acquisition represents the 14th internal medicine transaction of the year.
