Confluent Health announced on October 14 a partnership with Commonwealth Hand & Physical Therapy. The transaction expands Confluent Health’s national footprint to more than 820 sites of care across the United States.
Commonwealth Hand & Physical Therapy is a Kentucky-based practice with six clinics across the state. The company’s certified hand therapists and physical therapists help patients with comprehensive care for upper extremity conditions, such as hands, wrists, shoulders and elbows, and physical therapy for neck, back, hip and lower extremities.

Confluent is a portfolio company of Swiss-based private equity firm Partners Group.
By joining forces, Commonwealth Hand & Physical Therapy will continue operating under its local brand while gaining access to Confluent Health’s management services, practice enablement resources and innovation ecosystem.
As part of the Confluent Health family, Commonwealth Hand & Physical Therapy will also benefit from unique growth and development opportunities, including access to the Confluent Health Scholars Program, which supports advanced education for clinicians nationwide. Financial terms of the deal were not disclosed.
According to data in the LevinPro HC database, this transaction represents the 37th Rehabilitation transaction of the year. Active acquires in the space this year include Ivy Rehab (four deals), Ivy Rehab for Kids (three deals), Sila Realty Trust (three deals) and U.S. Physical Therapy, Inc. (three deals). There were 46 Rehabilitation acquisitions announced in 2024.
This also marks Confluent’s first acquisition of 2025. The company announced two deals in 2024; it acquired Access Physical Therapy & Wellness in May, followed by Fitness Quest Physical Therapy in September. Confluent has announced 26 Rehabilitation deals since 2019.
