On October 2, Avenues Home Care announced its expansion into Alabama through the acquisition of As Close As Family (ACAF) in Rainsville, Alabama.
ACAF operates multiple locations, providing compassionate home care throughout Alabama. The company has affiliations with the Home Care Association of America and the Alabama Elder Care Planning Council.
Avenues Home Care provides personalized homecare services for adults, seniors and veterans. The company has 14 locations across Texas, Tennessee and Georgia. Avenues Home Care was acquired by Nashville, Tennessee-based private equity firm Capital Alignment Partners in March 2024.
Local leadership and caregivers of ACAF will remain in place, ensuring continuity of care and preserving the trusted relationships built across Alabama communities.
“The team at As Close As Family knows these communities better than anyone,” said Doug Markham, CEO of Avenues Home Care. “Our job is to protect that local trust and deepen community ties by equipping our Alabama team with the support they need to keep delivering reliable, compassionate care.” Financial terms of the deal were not disclosed.
With this acquisition, Avenues Home Care now serves clients in four states: Texas, Tennessee, Georgia and Alabama. This also marks Avenues Home Care’s second acquisition since it was acquired by Capital Alignment Partners. In December 2024, the company acquired Clear Path Home Care in Denton, Texas.
According to data captured in the LevinPro HC database, this marks the 77th Home Health & Hospice (HH&H) transaction announced in 2025. There were 96 HH&H deals announced during full year (FY) 2024, 100 announced during FY 2023 and 116 announced during FY 2022.
