Choice Health at Home ended the month with two deal announcements, both transactions reported on January 30, 2026.
The first announcement was the acquisition of Cy-Fair Health Care and Alliant Home Health, Palliative, and Hospice Care.
Cy-Fair Health Care is located in Tomball, Texas, and provides home health services throughout Southeast Texas. Alliant Home Health, Palliative, and Hospice Care provides both home health and hospice services. The organization operates two offices in Colorado, one in Westminster and one in Colorado Springs.
Patients, families and referral partners can expect continuity of care throughout the transition. Existing staff will remain in place, and services will continue without disruption. Over time, both organizations will benefit from Choice Health at Home’s clinical expertise, operational support and commitment to education, innovation and employee development. Financial terms of the deal were not disclosed.
The second announcement was the purchase of Senior Nannies (Senior Nannies Home Care Services and Senior Advantages Assisted Living Placement Services), one of the largest personal care operators in the Southeastern United States. The transaction marks Choice’s entry into the Southeastern United States, establishing Florida as a strategic anchor and creating an integrated personal care platform to support broader regional expansion.
Choice Health at Home, a portfolio company of Coltala Holdings and backed by Trive Capital, was founded in 2008 as a rehabilitation service provider, entered home health in late 2012 and launched its hospice segment in 2018. The company provides a broad spectrum of post-acute services including home health, hospice, home care, remote patient monitoring and rehabilitation services.
Financial terms for both deals were undisclosed.
Choice Health at Home announced two deals in 2025, both also in January. These deals included Houston, Texas-based Family Tree Private Care and Conroe, Texas-based Devotion Hospice.
According to data captured in the LevinPro HC database, there have been 11 deals announced in the Home Health & Hospice (HH&H) sector so far in 2026. There were 111 HH&H deals announced during full year (FY) 2025, 98 announced during FY 2024 and 100 announced during FY 2023.
