Community Hospice of South Alabama is joining ACG Hospice, part of the Agape Care Group family of providers. The deal will grow care resources and expand clinically excellent hospice care throughout Alabama. 

Community Hospice of South Alabama is a hospice agency providing community-based home care services to terminally ill patients in Foley, Bay Minette and Mobile, Alabama. 

As a leader in hospice care, ACG Hospice serves patients across Alabama, Kansas, Louisiana, Mississippi, Missouri, Oklahoma and Virginia.

With the three Community Hospice branch locations, ACG Hospice will now provide care in 41 counties through 11 branch locations in Alabama. In January 2026, Community Hospice will change its name to ACG Hospice. Financial terms of the deal were not disclosed. 

According to data captured in the LevinPro HC database, this marks the 93rd Home Health & Hospice (HH&H) transaction announced in 2025. There were 98 HH&H deals announced during full year (FY) 2024, 100 announced during FY 2023 and 116 announced during FY 2022.
