Private equity-backed Choice Health at Home announced that it is acquiring the Oklahoma City-based Accentra Home Health and Hospice.

Accentra Home Health and Hospice is dedicated to providing home health and hospice services to patients across Oklahoma. Accenture’s growing network can admit patients in the cities and towns surrounding Oklahoma City, Tulsa, Kingfisher, Henryetta, Norman, Duncan, Chickasha and Enid.

Choice Health at Home, a portfolio company of Coltala Holdings and backed by Trive Capital, was founded in 2008 as a rehabilitation service provider, entered home health in late 2012 and launched its hospice segment in 2018. The company provides a broad spectrum of post-acute services including home health, hospice, homecare, remote patient monitoring and rehabilitation services.

Choice Health at Home also announced that it has closed on $260 million of new leveraged capital to continue expanding throughout the Southwestern United States. The acquisition of Accentra Home Health will significantly bolster Choice’s Oklahoma footprint. Following integration, Accentra will be able to cover 90% of the state’s urban and rural geographies. Accentra will also eventually rebrand to Choice Health at Home. Financial terms of the deal were not disclosed.

According to data captured in the LevinPro HC database, this acquisition marks the 80th Home Health & Hospice (HH&H) transaction of the year. There were 100 HH&H deals announced during 2023, and 115 announced during 2022.

Additionally, this marks Choice Health at Home’s fifth deal since it received an investment from Trive Capital in August 2021. The company acquired Kindful Hospice & Amed Home in January 2022, Texas assets of Abiding Home Health in April 2022, Instant Care of Arizona in October 2022 and Lumicare Hospice in November 2023.
