Chicago-based PurposeCare is expanding with the acquisition of three Midwestern home-based care agencies, Scott’s Home Healthcare and Attentive Personal Care, which are both located in Indiana, as well as Choice Nursing & Home Health in Ohio.

Scott’s Home Healthcare is a family owned healthcare service provider for individuals and families in and around Kokomo, Indiana. The company specializes in Medicare skilled nursing in-home health services.

Attentive Personal Care is an in-home care provider serving Indianapolis, Indiana and the surrounding area. Attentive Personal Care offers non-medical services in the home, such as meal preparation, housekeeping and transportation.

Based in South Euclid, Ohio, Choice Nursing & Home Health provides skilled nursing care and non-medical home health aides to patients in their home.

Established in December 2021, PurposeCare provides home-based care to more than 3,000 clients each month. PurposeCare is a portfolio company of Lorient Capital, a private investment firm focused exclusively on the healthcare industry.

The new acquisitions fit into PurposeCare’s existing portfolio in terms of service offerings, geography and payer mix. Financial terms of the deal were not disclosed.

According to data captured in the LevinPro HC database, this transaction represents the 47th Home Health & Hospice acquisition of 2023. This also marks PurposeCare’s second deal of 2023. The company acquired Home Sweet Home In-Home Care in March for an undisclosed price.
